Free Athletic Physicals

Rising 6-12 grade students who participate in athletics are required to have a valid GIAA physical form on file at school. 
 
Click here (link coming soon) to download the participation permission and physical forms. Students should bring the SIGNED permission and physical forms with them and the top half of the first page of the physical form must be completed prior to the physical.
Submitting your physical form:
Completed physicals are to be uploaded to Magnus Health via the Lion Portal.
